- Declaration of Acceptance
- Magnet Enrollment Agreement (Form A)
- Media Release Form
- 9th Grade Subject Selection Form (only incoming 9th graders)
- 10th Grade Subject Selection Form (incoming 10th graders only)
Non-MDCPS students must submit additional documentation. Please contact our school’s Registrar, Ms. Mills, via email at lmills@dadeschools.net.
- Registration Form
- Original birth certificate or birth card – Must be original; hospital certificate not acceptable.
- Student Health Examination – DH 3040 yellow form health examination performed within one year prior to enrollment
- Florida Certificate of Immunization – DH 680 blue card from a private doctor or local health provider
- Transcripts
