As we prepare for a possible physical reopening of school and the community reassess the local risk, many parents are contacting us to inquire about the process of change to the instructional delivery model selected for their child in July 2020. Based on these results, a schedule was created and we are the point to reevaluate requests on a case-by case basis to determine the potential for physical accommodation at the school site setting.

The proper instructions must be followed to ensure your child is placed in an internal placement list based on delivery model preference, date and time form was received, and determining factors for the availability of schoolhouse slots for each of the courses currently on the student’s schedule.
- Open attachment, complete fillable sections, save as (ID#, Student Name) Example: 123456 Smith, John, then email form to PR7731@dadeschools.net
- Come by the school’s Main Office, complete the form, and submit to school’s registrar.
The request will be reviewed by the school’s administration to determine the school’s ability to accommodate the change safely.
A notification will be sent to parents regarding their request no later than October 23rd, 2020.
