PAYMENT METHOD: Cash or Money Order made payable to Miami Southridge Senior High (do not send cash via mail).

To order, please include the following information:

1. Name while attending Southridge
2. Date of Birth
3. Student ID Number (if you remember it) or Social Security Number
4. Graduation Date or date of last attendance
5. Phone Number
6. Photo ID (may be scanned and sent via e-mail)
7. If the transcript/diploma will be picked up or mailed
8. If mailed, include the complete name and address where it is to be mailed.
